OIS specialist staff facilitate and harmonize the application and payment process with the Embassy of Nigeria. However, OIS Washington Metro office does not currently process passport applications. Many applicants rely on our experienced service professionals, business centers and online resources to ease the passport application process. Applicant will be required to appear in person at the Embassy for biometric and image acquisition.

Below are different checklists that detail the mandatory items and documents you will need to present with your completed passport application form. To read through a checklist of the requirements for your passport application, simply click on the relevant link below. You will also be provided with the option to download or print out a copy of your chosen checklist.

After you have confirmed the requirements for your passport, please proceed to the Nigeria Immigration service`s portal at https://portal.immigration.gov.ng/pages/welcome to complete online application forms as well as to print the Acknowledge Slip and Receipt after making payment




PASSPORT CATEGORY

  • REQUIREMENTS FOR CHANGE OF PHOTO

    • At the Nigerian Embassy only


  • REQUIREMENTS FOR CHANGE OF NAME AS A RESULT OF DIVORCE FOR APPLICANT WITH NIGERIAN PASSPORT (WOMEN ONLY) BEFORE APPLYING FOR YOUR NEW NAME TO REFLECT ON YOUR E-PASSPORT –

    • Original and certified photocopy of your divorce decree and marriage certificate.

    • Print out of online payment confirmation slip.

    • Print out of online payment acknowledgement slip.

    • Presentation of old Nigerian passport or current e-passport.

    • Postal order as fee for change of name endorsement for old passports.

    • Postal order processing fee. All payable to Embassy of Nigeria.

    • Appointment booking slip.

    • Change of name by court order is not applicable

  • REQUIREMENTS FOR STANDARD E-PASSPORT FOR MINORS UNDER 18 YEARS (FIRST ISSUE)

    • Original & copy of birth certificate containing details of parents.

    • Letter of consent signed by both parents.

    • Photocopy of data page of  parents’ current Nigerian passport. One of the passport data pages must be a Nigerian Passport. .

    • Photocopy of current data page of non-Nigerian parent.

    • Print out of online payment confirmed slip and acknowledgement slip.

    • $30.00 Postal order processing fee per applicant payable to the Embassy of Nigeria.

    • If born outside the United States, please submit a copy of the appropriate documents including mother’s passport where the child is endorsed. If any document you are providing is in a language other than English, provide an official translation. This must be signed and stamped by official translator to prove it is genuine.

    • Appointment booking slip.

    • Adoption decree in applicable

    • These requirements are for each child.

    • NOTE: These are essential requirements. If you are without these documents your child’s application will not be processed. Please bring ALL original documents accompanied by copies.


  • REQUIREMENTS FOR ADULTS WITHOUT NIGERIAN PASSPORT OVER 18 YEARS (FIRST ISSUE)

    • Print out of completed online passport application form.

    • Print out of online passport application confirmed payment slip.

    • Print out of online application acknowledgement slip.

    • Copy of current passport (any nationality).

    • Copy of data page of the (Nigerian) parent.

    • Original & copy of birth certificate full version (showing details of parents).

    • $30.00 Postal order processing fee payable to the Embassy of Nigeria.

    • In the event of death of either parent, a copy of the death certificate must be presented.

    • Appointment booking slip.

    • Change of name by deed poll is not applicable.





  • REQUIREMENTS FOR ISSUANCE OF E-PASSPORTS TO HOLDERS OF OLD NIGERIAN PASSPORTS

APPLICANTS ARE REQUIRED TO COME ALONG WITH THE FOLLOWING DOCUMENTS ON YOUR APPOINTMENT DATE:

    • Your old Nigerian passport(s).

    • In case of dual nationality, present the other passport(s).

    • Printout of completed online passport application form.

    • Printout the online passport application confirmed payment slip.

    • Printout of online application acknowledgement slip.

    • $30.00 Postal order processing fee payable to the Embassy of Nigeria.

    • Appointment booking slip.

    • Change of name by deed poll is not applicable.

  • REQUIREMENTS FOR STANDARD E-PASSPORT FOR MINORS IN THE CASE OF A SOLE CUSTODIAN

    • *Proof of sole legal custody of child/children by court order.

    • Letter of consent from the sole custodian.

    • Copy of data page of the sole custodian’s current Nigerian passport.

    • Child’s birth certificate containing parent’s details.

    • Print out of online payment confirmed slip and acknowledgement slip.

    • $30.00 Postal order processing fee payable to the Embassy of Nigeria.

    • If born outside the US, please submit a copy of the appropriate documents including mother’s passport where the child is endorsed.   • REQUIREMENTS FOR STANDARD E-PASSPORT FOR ADOPTION (MINORS)

    • *Proof of sole legal adoption documents of child/children from a U.S court.

    • Letter of consent from the adopting parents.

    • Child’s birth certificate if possible.

    • Print out of online payment confirmed slip and acknowledgement slip.

    • Postal order processing fee payable to the Embassy of Nigeria.

    • If born outside the US, please submit a copy of the appropriate documents including mother’s passport where the child is endorsed. If any document you are providing is in language other than English, please provide an official translation. This must be signed and stamped by an official translator to prove it is genuine.

    • Change of name by deed poll is applicable. (Only to the name of the adopting parents).

    • Appointment booking slip.

    • These requirements for each child.

    • NOTE: These are essential requirements. If you are without these documents your child’s application will not be processed. Please bring ALL original documents accompanied by copies.

  • REQUIREMENTS FOR RE-ISSUE OF STANDARD E-PASSPORT             Standard E-passport may be re-issued due to one of the following reason ONLY:

    • Filled up pages on the current E-Passport.

    • Change of name by marriage (FOR WOMEN ONLY).

    • When the life span of the E-passport has elapsed (due to expiry of the passport).

    • PASSPORT AMENDMENT:  Changes or data corrections on your Nigerian e-passport can only be done in Nigeria. The Embassy no longer entertains such request.

  • REQUIREMENTS FOR RE-ISSUE OF STANDARD E-PASSPORT DUE TO CHANGE OF NAME BY MARRIAGE (WOMEN ONLY)

    • Original and photocopy of marriage certificate.

    • Print out of online payment confirmation slip.

    • Print out of online payment acknowledgement slip.

    • Presentation of the old e-passport.

    • $30.00 Postal order payable to the Embassy of Nigeria.

    • Photocopy of husband’s current passport data page.

    • Letter of consent written and signed by husband consenting to change his name.

    • Appointment booking slip.

    • Change of name by deed poll is not applicable.

    • Note: These are the essential requirements for the change of name by marriage. If you are an e-passport holder and you cannot supply all these documents, your e-passport will not be issued.


  • REQUIREMENTS FOR CHANGE OF NAME BY MARRIAGE FOR APPLICANTS WITH OLD NIGERIAN PASSPORTS (WOMEN ONLY) BEFORE APPLYING FOR YOUR NEW NAME TO REFLECT ON YOUR E-PASSPORT

    • Original and photocopy of your marriage certificate.

    • Print out of online payment confirmation slip.

    • Print out of online payment acknowledgement slip.

    • Presentation of old Nigeria passport.

    • Letter of consent written and signed by husband consenting to change to his name.

    • Photocopy of husband’s current passport data page.

    • $30.00 Postal order processing fee. All payable to the Embassy of Nigeria.

    • Appointment booking slip.

    • Change of name by deed poll is not applicable.

  • REQUIREMENTS FOR REPLACEMENT OF LOST PASSPORTS

  • REQUIREMENTS FOR ENDORSEMENT OF CHILD ON PARENT’S PASSPORT (FOR CHILDREN UNDER 5 YEARS AND ONLY ON OLD MRP PASSPORT)

    • Children can now have separate electronic passports.

  • REQUIREMENTS FOR EMERGENCY TRAVEL CERTIFICATES (ETC)

    • Certificate is issued for emergency purposes only.

    • ETCs are issued to facilitate applicant’s immediate one-way departure for Nigeria only.

    • Two passport photographs of the applicant.

    • Police report if it’s stolen or missing.

    • A guarantor’s form together with the guarantor’s passport copy.

    • Utility bill from the guarantor.

    • Postal order payable to Embassy of Nigeria (5 days).

    • Postal order payable to Embassy of Nigeria (Express 24 HRS).

    • In the event of an expired passport, a copy of the passport is required.

    • Ensure completion of form C1.

    • Issued to non-residents who are without Nigerian passports only.

    • Change of name by deed poll is not applicable.

    • Travel appointment is required.
