• Letter of consent signed by both parents.
  
• Photocopy of data page of both parents’ current Nigerian passport.
  
• Photocopy of current data page of non-Nigerian parent.
  
• Printout of completed online passport application form.
  
• Print out of online payment confirmed slip and acknowledgement slip.
  
• If born outside the UAE, please submit a copy of the appropriate documents including mother’s passport where the child is endorsed. If any document you are providing is in a language other than English, provide an official translation. This must be signed and stamped by official translator to prove it is genuine.
  
• These requirements are for each child.

REQUIREMENTS FOR ISSUANCE OF E-PASSPORTS TO HOLDERS OF OLD NIGERIAN PASSPORTS & EXPIRED E-PASSPORTS

 APPLICANTS ARE REQUIRED TO COME ALONG WITH THE FOLLOWING DOCUMENTS ON YOUR APPOINTMENT DATE:

• Your old Nigerian passport(s).
  
• In case of dual nationality, present the other passport(s).
  
• Printout of completed online passport application form.
  
• Printout the online passport application confirmed payment slip.
  
• Printout of online application acknowledgement slip.
• Proof of Residency in the UAE
